401(k) Retirement Program
Maximizing your retirement no longer means piecing together and managing several plans. With Clover, you can plug into one of the most powerful retirement programs available anywhere. You may contribute up to the legal limit in 2012, that's $17,000 (or $22,500 if you're over 50). It's the easiest and most flexible way to provide for your future!
Base, HSA Plus and Gold Medical Plans
The Aetna Preferred Provider Organization (PPO) plans pay benefits for covered services obtained from any heath care provider. The level of benefits is higher for services from participating providers. If you need a medical specialist, you can go directly to that specialist. This year Clover will offer three (3) PPO medical plan options including a high-deductible health plan with a Health Savings Account (HSA) option administered by Aetna. The goal of our medical plans is to provide competitive quality benefits for employees and covered dependents in the most cost-effective manner.
Flexible Spending Account
Contribute up to $1,000 annually with pre-tax payroll deductions to reimburse yourself for eligible medical expenses not otherwise covered. Sign up for the Debit Card. Purchase prescriptions or pay for a doctor’s office visit co-pay with a debit card where the monies are directly taken from your flexible account. NO MORE CLAIM FORMS!
Voluntary Life, Short and Long Term Disability Plans
With group term life insurance, you are protecting and providing for your family in the unfortunate event of your death. Clover's life insurance plan is voluntary. Short Term Disability insurance will provide payments to replace up to 60% your weekly income or $1,500/week, whichever is less, while you are unable to perform your duties of your job due to a covered disablement (or sickness) beginning on the 7th day (for up to 25 weeks). Long Term Disability is income replacement of up to 60% of your monthly income or $5,000, whichever is less, for disabilities that last longer than 180 days.
Dental Plan
Principal Dental is a fee-for-service (or indemnity) plan that gives you complete freedom in selecting a dentist. You can visit any dentist in the phone book. All dentists will honor this plan. If you select a dentist from the Principal network, you can take advantage of negotiated discounts.
Vision Plan
The OptiCare Vision Plan is a comprehensive managed care program. It is a full-service plan where members receive an exam, lenses or contacts annually and a frame of their choice ($125 allowance) every 2 years.
Voluntary Plans
Clover has Cancer, Accident, Critical Illness and Hospital Confinement insurance plans that are offered to employees and pay in addition to other insurance coverage that may also be in place.
